LPN/LVN (Home Health)-
Location: Lexington, KY 40505
Pay: $42/hour
Schedule: 40 hrs./week, rotating weekends; possible on-call
Position Overview
We are seeking an LPN/LVN to provide skilled nursing care in patients' homes under the supervision of an RN Case Manager. You will support patient assessments, coordinate care with the interdisciplinary team, and document care in compliance with State, Federal, and Medicare standards.
Key Responsibilities

• Provide skilled nursing care in the home per physician orders and plan of care
• Identify patient physical, psychosocial, and environmental needs through observation and documentation
