Our client is not-for-profit social service organization in Singapore.
They are seeking a dynamic and design-thinking Manager to join their team as part of new expansion. This newly created role will help ensure the organization acts as a single point of contact and champion seamless coordination between various long-term care services through unified assessment, thereby reducing duplication and improving care continuity for its clients.
Key Responsibilities
Champion care coordination and integration by drafting and refining the processes that allow seniors to move seamlessly across community care settings.
Coordinate with caregivers, next-of-kin, and other external stakeholders to strengthen service linkages and ensure continuum of care
Develop and review service manuals, existing SOPs, and communication protocols to drive continuous improvement in care delivery.
