Medical Social Worker

The role involves providing psychosocial care across a range of eldercare community settings within the organization, including homecare, home hospice, and Centre-based services, as part of an integrated model of care.
Role & Responsibilities
Conduct psychosocial and financial assessments for patients/clients and their caregivers
Provide direct counselling, emotional support, and psychoeducation to patients/clients and their families to facilitate coping with illness, grief, loss, and life transitions.
Collaborate with the multi-disciplinary team (MDT) to develop individualized care plans and facilitate family conferences to support communication and shared decision-making.
Act as an advocate and liaison for patients/clients and their families, liaising with internal and external stakeholders, community partners, and government agencies to ensure access to necessary resources and services.
