Key Responsibilities

Chart Prepping & History Review

• Review next-day schedules and prepare patient charts
• Pull forward key problems, diagnoses, and relevant history from prior visits
• Summarize the patient's background into a clear 1–2 paragraph snapshot that can be reused across notes
• Identify and highlight outstanding labs, imaging, and consult reports that need provider attention

Note Creation & Scribing

• Work from AI-generated transcripts and drafts (Granola and EMR tools) to create accurate, well-structured SOAP notes
• Integrate patient snapshot, HPI, exam, assessment, and plan into a cohesive clinical note
• Assign appropriate ICD-10 diagnoses
• Ensure documentation is clinically sound, complete, and audit-ready
• Collaborate closely with the physician to refine notes and maintain a consistent documentation style
