We have an immediate Opening for a Meds Only Case Manager.
Key Responsibilities:
+ This individual will be working directly with children and their families in the community.
+ This individual should be familiar with Targeted Case Management and be highly organized to complete assessments, care plans, and monitor the patient's needs.
+ Flexible work schedule is required for this position to ensure continuity of care.
Required Skills/Education:
+ Bachelors or Master’s Degree in human services, social work, or counseling to work with children 0-18 and their families in the home, community, and school.
