Nurse Counsellor

We are seeking a Registered Nurse to work with the group of general practitioners (GPs) in the network to provide structured nurse counselling, health screening services, patient education on chronic disease, mental health management and health promotion through face-to-face or virtual consultations.

Responsibilities

• Work with the group of general practitioners (GPs) in the network to provide structured nurse counselling, health screening services, patient education on chronic disease, mental health management and health promotion through face-to-face or virtual consultations
• Identify patients suitable for nurse counselling to help them understand their chronic conditions and improve their health outcomes
• Assess patients’ social history, current lifestyle and dietary habits to help patients bring about effective change and enhance their well-being
