Nurse Home Visitor

Our subsidiary's Nurse Home Visitor is responsible for making home visits to mothers enrolled in the Nurse Family Partnership (NFP) program. This position works with the mother and her child while eligible for services under the program. These services include providing care coordination plus working with the mother and the child to provide some basic services such as blood pressure monitoring and weighing the child. Case documentation is recorded to document the visits for billing and record of services. This position will be required to network with community resources and the medical community to integrate our program into the right resources in local communities. This position must be able to interact with and direct the efforts of external stakeholders in order to be successful.
