**Job Summary and Qualifications**

**The Total Joint Program Nurse Navigator (TJNN) is responsible for overall coordinating and implementing the center's Total Joint Replacement Program. The TJNN will be guiding the total joint patient and their family through the program in all aspects of the care continuum from preoperative assessment and education through discharge and follow up. The TJNN will also complete an assessment of the patient’s current support system and their ability to return home after surgery. The TJNN will act as a liaison between all d...
