The Opportunity

Robinvale District Health Services' is seeking an experienced and innovative Nurse Practitioner to join our dynamic Community Health team. In this full-time role, you will be responsible for providing comprehensive primary healthcare services to the diverse population of Robinvale, Victoria.

Key Responsibilities

• Assess, diagnose, and manage a wide range of acute and chronic health conditions in the community setting
• Provide health education, promotion, and disease prevention services to individuals and groups
• Collaborate with other healthcare professionals to develop and implement holistic care plans
• Participate in the development and delivery of community-based health programs
• Contribute to the ongoing evaluation and improvement of community health services
• Maintain accurate and comprehensive patient records in accordance with professional and organisational standards
