Nurse Practitioner - Elevate Medical Clinic (Campbell River PCN) 300-1026 (Nurse Practitioner)

Elevate Medical Clinic is looking for a 1.0 FTE Nurse Practitioner to join its team. The clinic provides specialized service in providing wrap-around care to those are marginalized and vulnerable and focus on low barrier access to primary care for those struggling with mental health, substance use issues, homelessness, and racism.

The clinic is a collaborative team of Nurse Practitioners and Family Physicians, including a Social Worker, Registered Nurses, and a Clinical Pharmacist. Many of the people who access care at our clinic face systemic barriers within the health care system and we work as a team to ensure we are reducing these barriers so people may access culturally safe and trauma-informed care.
