FNP Per-visit Role

FNP Per-visit role, 8hrs per week (you build your schedule); $145.00/in-home (includes mileage) & $85.00/telehealth. Medicare/Medicaid enrollment required to conduct Annual Risk Assessments with geriatric population + Chronic Condition Verifications with pediatric population.
