Note: 1+ year of professional experience as a Family Nurse Practitioner in either a family medicine Primary care practice, or urgent care required.

Schedule: Must be able to work at least 2-3 days per week, 830a-730p. Additional hours may be available. Training is 4 ten or 5 eight-hour shifts for four weeks (but flexible).

Job Summary:

• Nurse Practitioners will work in collaboration with a dedicated clinical care team to provide evidenced-based care to a panel of patients.

• The clinical care team will work with the provider daily to drive better outcomes for entire panel of patients.
