Salary Range: Annualized salary range $121,119.00 - $125,916.00 commensurate on skills and experience, plus participation in HOOPP.

Hours of Work/Schedule: 35 hours a week, Monday to Friday 8.30am – 4.30pm, includes home visits

Position Reports to: Senior Manager, Integrated Care and Experience

Primary Location: Oshawa (with flexibility to transition to/work at other DCHC sites as required).
