Nurse Practitioner (Part-Time)

Position Summary

This is a vacant position. The Nurse Practitioner (NP) is responsible for leading the provision of comprehensive geriatric services as part of a collaborating member of an inter‑professional Geriatric Assessment Intervention Network (GAIN) Community Team. The NP provides direct patient care focusing on health promotion, maximizing patient safety and function to support frail seniors living at home.
