Outpatient Case Manager - Adult

POSITION SUMMARY:

The Adult Outpatient Case Manager provides case management services and support to an assigned caseload of adult clients receiving outpatient mental health services. They are responsible for assisting clients with case management and care coordination needs that are goal-oriented and individualized.  They develop an integrated treatment plan in collaboration with the client and other service providers as well as any natural support the client wants to participate in.

 

EDUCATION AND EXPERIENCE:


+ Minimum if a bachelor’s degree in a mental health, or related field, from an accredited school.

+ Minimum of 1 year of case management related experience or combination of field practicum and experience.

+ Prior experience working in a Community Health setting preferred.
