Your Opportunity:
The Outreach Worker will engage in case management, system navigation, and provide direct support by connecting with patients in the community and in their homes. You will work closely with the Social Worker to develop, implement, and support the execution of personalized care plans, accompany patients to appointments, and ensure that they receive the care they need.
