Part-Time Outreach Physician Position

Be part of a compassionate outreach team as a Part-Time Physician at the Health Centre. Focus on individuals experiencing homelessness and complex health needs while supporting mobile care teams.
The Health Centre is looking for a dedicated outreach physician for part-time work (FTE 0.6 or 0.8). You will assess and treat vulnerable patients while addressing their mental health and addiction needs. Your role involves collaboration with interdisciplinary teams and contributing to public health initiatives in Kitchener.
Key Responsibilities:
• Perform assessments and provide diagnosis and treatment
• Work to improve health outcomes via Social Determinants of Health
• Aid in addressing addiction and mental health challenges
• Triaging client needs through various modalities
• Participate in health promotion and disease prevention programs
Requirements:
• Medical degree with CFPC Certification or equivalent
• Minimum three years of community-based experience
