Role Overview:
We are looking for a dedicated Care Coordinator with a strong healthcare background or clinical experience to join our healthcare team. The ideal candidate has hospital or clinical experience and/or preferably has worked in case management or transition of care. This role requires working onsite at our Bacolod office and on a night shift schedule.

• Manage and coordinate chronic care plans for patients with long-term health conditions.
