Patient Care Coordinator

About the Role

We're looking for a compassionate and driven Registered Nurse to join our care management team. In this role, you'll be the connective tissue between our most complex patients and the care they need — building ongoing relationships, closing gaps in care, and reducing avoidable hospitalizations across our ACO programs.

What You'll Do

• Identify and maintain ongoing communication with complex medical patients enrolled in our ACO programs.
• Help patients navigate care: coordinate referrals, assist with appointment scheduling, and proactively work to prevent unnecessary ER visits.
• Identify Medicare patients eligible for Annual Wellness Visits and manage outreach accordingly.
