**Salary:** $90,000–$110,000 annually (based on experience)

Are you a Registered Nurse with home health leadership experience? We are seeking an experienced **Patient Care Manager** to oversee clinical operations, support field staff, and help ensure exceptional patient care in a home health setting.

**Responsibilities**

+ Provide clinical oversight and supervision of patient care services
+ Coordinate and monitor patient care plans and service delivery
