Job Summary:

A Patient Care Navigator help patients navigate the healthcare system by ensuring that the healthcare system fulfills the demand of the patient as best as possible.

Patient care navigator examine, plan and advocate for the patient's health needs. S/he monitor and coordinate patients' treatment plans, help them understand their health state, and follow up on their progress.
