Patient Coordinator

Role Description
We are looking for a Patient Coordinator to join our team and help ensure patients receive timely access to their medications. This role is ideal for someone who is organized, proactive, and enjoys communicating with both patients and internal teams to keep every case running smoothly.
Key Responsibilities
Provide customer support to patients, office and pharmacy team.
Track and follow up on patient cases to ensure timely access to medications.
Communicate with the necessary teams to coordinate approvals, orders and next steps.
Keep all internal teams aligned and ensuring every case stays on track.
Share clear updates with patients and internal teams regarding the status of prescriptions and authorizations.
Maintain organized case tracking and ensure no patient request is left unresolved.
Support the Patient Coordination Lead with scheduling, reminders, and follow-ups to guarantee continuity of care.
