Job Overview

The posts available are 1x36 hrs, 1x32 hrs and 1x24 hrs. These posts will be based within the (Fife-wide) Integrated Discharge Team, mainly within Victoria Hospital, Kirkcaldy, and on occasion may be asked to cover Community Hospitals across Fife. This post involves comprehensive assessment and discharge planning.

Key Responsibilities

We work to support discharge to meet PDD. Our ethos is patient‑centred pathways within the Home First Model. Attending ward daily MDTs, completing assessments and communicating with the relevant teams across the Partnership and third‑sector organisations.

The role requires an enthusiastic individual who takes a dynamic approach to complex discharge planning. Ideally a breadth of experience working within Health and Social Care would be desirable.
