Patient Flow Coordinator

These posts will be based within the (Fife-wide) Integrated Discharge Team, mainly based within Victoria Hospital, Kirkcaldy and on occasion may be asked to cover Community Hospitals across Fife. This post involves comprehensive assessment and discharge planning.
We work to support discharge to meet PDD. Our ethos is patient centred pathways within the Home First Model. The post involves attending ward daily MDT's completing assessments and communicating with the relevant teams across the Partnership and Third Sector organisations.
The role requires an enthusiastic individual who takes a dynamic approach to complex discharge planning. Ideally a knowledge and wide range of experience working within Health and Social Care would be desirable.
