seeking a full-time Patient Outreach and Enrollment Specialist that can work from Edge Campuses
to provide essential clinical support and population health management. Reporting directly to the Director of Clinical Effectiveness, this position functions as a vital link supporting care teams to ensure patients, providers, and clinics are thoroughly prepared for high-quality, efficient visits.
