**What you will do:**

Provide direct patient care to individuals participating in the CardioPulmonary Rehab and Wellness programs at both Albany and Troy locations.

**Responsibilities** :

+ Participate in the development of individualized treatment plans.
+ Provide patient education.
+ Assist patients with exercise equipment and ensure safety.
+ Assess and reassess patient’s needs to achieve both program and personal goals.
