Personal Support Worker (Homecare) – NF 22/26

Job Summary:
The Personal Support Worker (PSW) is a member of the interdisciplinary healthcare team responsible for providing direct personal care and support in accordance with patient care plans and the direction of the Patient Care Manager or supervising RPN. As part of Ontario Health’s Hospital to Home (H2H) program, the PSW supports patients transitioning from hospital to home, including Emergency Department patients and inpatients designated as Alternate Level of Care (ALC) or at risk of becoming ALC, who require restorative, reactivation, rehabilitation, or optimization

care services. Working collaboratively with other healthcare disciplines, the PSW promotes safe and timely discharges, continuity of care, quality of life, independence, emotional well-being, and participation of family and community while incorporating rehabilitative principles and patient-centered care approaches.
