Job Overview

The PSW/HCA/CCA provides direct personal support and care to patients, meeting their physical, intellectual, emotional, social, and spiritual needs in line with policies and procedures.

Responsibilities

• Assist with ambulation, feeding, bathing, grooming, and other personal care tasks


• Follow nursing care plans and documentation protocols


• Communicate positively with staff, residents/patients, and families


• Provide care in accordance with applicable legislation and job routines

Qualifications

• Completed a PSW Program or HCA/CCA certification meeting required standards


• Experience in long‑term care or community home care, preferably with dementia and palliative care exposure


• Knowledge of care planning systems and documentation
