Job Description

Key Responsibilities:

✔ Conduct medication reconciliation and dispensing to ensure safe and effective medication use.
✔ Provide specialized counselling to patients and conduct medication reviews to optimize treatment plans.
✔ Support hospital projects and initiatives to enhance pharmaceutical care.

Requirements:

✅ Recognized Bachelor of Science (Pharmacy) degree.
✅ Registered with the Singapore Pharmacy Council and hold a valid practising pharmacist license.
