Physical Therapist

Provides direct physical therapy to patients in their residence. Receives a physician order for therapy and develops and implements plan of care. Provides direct supervision of qualified physical therapy assistant and evaluates effectiveness of services provided by the assistant. Adheres to the Code of Conduct and upholds the philosophy, vision, and core values of the organization and ensures the Christian Mission of EverTrue is realized so that Older Adults Live Life to the Fullest.

Essential Duties and Responsibilities:

1. Assesses patient functional ability and potential. May perform the initial comprehensive assessment in those instances when the doctor only orders physical therapy services

2. Completes PT evaluation to determine potential benefit from therapy and to establish an effective plan of treatment

3. Develops and implements medically prescribed PT plan of treatment
