Physical Therapist II - Outpatient Orthopedics

We are currently seeking a*Physical Therapist II*to join our outpatient ortho team at our Community Clinic locations (this role will be primarily at our Brooklyn Park Clinic location, with potential to train at or and cross cover to other locations). This full-time role will work 5 days per week (Monday to Friday, either 8:00 AM to 4:30 PM or 8:30 AM to 5:00 PM).

*Purpose of the position:*In collaboration with an interdisciplinary team, provides therapeutic services including evaluation, treatment, discharge planning and education.

* Performs and documents comprehensive physical therapy evaluations
* Develops and documents patient care plans to promote optimal function and safety
* Observes, evaluates, and documents patient's progress
* Participates in hospital rounds and patient care conferences
