Salary range:$182.40 per hour or $331,973 per year (prorated to your FTE), (along with HOOPP pension plan, benefits, depending on eligibility)

Reporting to:Medical Director and VP of Community Health
Locations:· Flemingdon Health Centre at 10 Gateway Blvd, or 5 Fairview Drive
· Health Access Thorncliffe Park (HATP) at 45 Overlea Blvd.
· Other Community sites
