About the Role
This position has two components of providing care coordination and physiotherapy in the community and also within the Emergency Department.

In the community, you will oversee a small caseload of clients with varied health and social needs in a person-centered approach. This role includes conducting comprehensive holistic assessments, coordinating health and social care needs, monitoring achievement of client goals, facilitating assessment and required intervention from other health care professionals to meet identified client needs, managing risk and ensuring appropriate linkages with hospital and community services.
