The Partner in the Community (PITC) Pathway Coordinator – First Nations supports people with disability across the consumer pathway, from referral through to discharge, including those transitioning out of or not eligible for the NDIS. As the first point of contact, the role provides timely advice, guidance, and practical support to individuals, families, and carers.
