The Post Majority Support Services Case Manager provides direct one‑on‑one support to clients who have reached the age of majority but need assistance and guidance as they transition into adulthood. This may include completing client intakes, assessments, and case planning. The Case Manager will ensure professional and administrative consultation and practice to ensure the success of delivering support services to clients of MKO First Nation communities. As a part of the PMSS team, the person will work under the Post Majority Support Services Coordinator; establish a safe atmosphere for the clients that require extra support and are at risk due to social, cognitive, medical, or home situations; be involved in the various activities dependent on the needs of the MKO First Nations communities in which they are supporting; participating in strategic planning activities...
