With a smaller patient panel, youll be able to dedicate the time and attention each participant deserves. What Youll Do Conduct thorough, in-person physical and wellness assessments. Collaborate with the Interdisciplinary Team to design and implement individualized care plans. Coordinate participant transitions between acute, short-term, and long-term care settings.
