Job Description
We're looking for a Production Medical Coder specialized in Evaluation & Management (E/M) services. This role focuses on high-volume, accurate coding across multiple specialties, ensuring revenue integrity through compliant code assignment, documentation clarification, and timely resolution of edits. Coders are expected to work independently while meeting productivity, turnaround time, and quality benchmarks.

Responsibilities
Perform daily coding for E/M, office-based, and procedural services.
Assign ICD-10-CM, CPT, HCPCS codes per CMS, NCCI, and specialty guidelines.
Apply appropriate modifiers (-25, -59, LT, RT).
Enter finalized surgical and graft codes into AdvancedMD (AMD).
Issue provider queries when documentation is unclear.
Adjust E/M codes based on documentation.
Escalate recurring issues to the Coding Manager.
Participate in audits and feedback processes.
