We are seeking a Registered Dietitian (RD) to join our Family Health Team. The RD will provide individualized nutrition counselling, support chronic disease management, and contribute to health promotion initiatives. This role involves both one-on-one patient care and group education, in collaboration with the broader care team.

Duties and Responsibilities

• Collaborate with patients to assess nutritional needs, considering dietary habits, medical history, cultural, socio-economic, and psychosocial factors.
• Conduct nutrition and anthropometric assessments to guide care planning.
• Develop, implement, and evaluate individualized nutrition care plans tailored to patient needs and context.
• Provide evidence-based nutrition counseling, diabetes education, obesity and weight management, and co-create self-management plans with patients.
