Job Description

                    Hospice Registered Nurse Case Manager 
Salary:  $80-89k/yr BOE 
Schedule:  M-F| 8:00 AM-5:00 PM + OC rotation 
Location:  Blue Ash, Ohio, and surrounding areas 
Requirements of the RNCM role:  
Licensed Registered Nurse  
BLS Certified  
Comfortable working in both private homes and facility settings.  
Valid Driver's License and Auto Insurance Coverage  
Reliable transportation  
A minimum of 1 year of Clinical experience  
Hospice experience preferred  
About the RNCM ro le:  Conduct comprehensive patient assessments, including vital signs and medical history review  
Travel to patients' residences to conduct visits  
Develop individualized care plans in collaboration with multidisciplinary teams
