As a central liaison between clinical teams, payers, and patients, your role assumes the responsibility and accountability for collaboratively managing the progression of patient's care with the physician and health care team utilizing evidence based best practice guidelines, a patient advocate who ensures optimal communication, appropriate use of clinical resources and management of patient flow.
What you will do :
Lead daily huddles to coordinate patient progression and address care barriers and work collaboratively with unit staff and support team (RN, Physician/Clinical Affiliate, DPA, MSW, CDI, Rehab Services, ...
