This exciting opportunity as Registered Nurse, Designated Float covers planned and unplanned staff absences. Integrated Home Care uses a strengths–based Client Centered Case Management practice framework. Using a person-centered approach you will develop a care plan supporting the client/family to identify needs, focus on strengths, set achievable personal health and self-care goals and provide direct nursing care. In collaboration with the client/family and interdisciplinary team you will ensure services are provided and modified on an ongoing basis, based on client’s response to interventions, incorporating stewardship into service authorization. You will report to the Home Care Manager and will be responsible for coordinating and providing quality care to clients in the community.
