Registered Nurse for Transitional Care

Provide compassionate care as a Registered Nurse in the CG-TCU at Sunnybrook Health Sciences Centre. Support patients in a diverse acute care environment while working under pressure in a dynamic setting.
This position requires a registered nurse to deliver person-centred care to patients awaiting inpatient beds in an acute care setting at Bayview Campus. Collaborate closely with an interprofessional team, including RPNs, to assess, plan, and implement high-quality clinical interventions while prioritizing patient needs and experiences.
