Registered Nurse II - Adult Community Clinical Services

The Department of Mental Health (DMH) is seeking dedicated, compassionate, and exceptional Registered Nurse who desire a rewarding career making a difference in the lives of individuals in the community. The Registered Nurse II serves as a member of the Adult Community Clinical Services (ACCS) Integrated Team; provides behavioral and physical health monitoring and support; Medication Administration Program oversite, coordination, and education; completes PHAs; participates in the development of IAPs; provides trauma-informed, person centered community-based interventions that target skill development in the areas of Health & Wellness; conducts routine assessments and provides medical care as needed; ensures that appropriate medical services are accessed by collaborating with care coordination entities. The selected candidate will function as the ACCS Team RN providing nursing oversight, consultation, and problem solving to the team's LPNs and MAP Certified team members.
