In accordance with the British Columbia College of Nurses and Midwives (BCCNM) standards of practice and the Mission and Values of Fraser Health, and working collaboratively and in partnership with the interprofessional care team in the provision of person-centered care, the Registered Nurse provides direct patient care by assessing, planning, providing and evaluating nursing care and by teaching patients and families about geriatric health issues, and illness management, prevention and treatment within a specialized seniors clinic setting. Responsibilities Assesses patients referred to a specialized seniors clinic (SSC) and collects information from a variety of sources using skills of observations, communication, data review, patient information received from referring health care providers, and physical assessment.
