Registered Nurse - Patient Aligned Care Team (PACT) - Outpatient Staff

Summary The VA New Mexico Healthcare System is seeking a Registered Nurse to serve on the Patient Aligned Care Team (PACT) - Outpatient Staff. They are responsible and accountable for all elements of the nursing process, including assessment, planning, implementation, and evaluation of age-specific care. Coordinates patient education and self-management, administers medications and procedures, collaborates with interdisciplinary teams, and demonstrates leadership in managing complex patient care. Responsibilities Duties include, but are not limited to: Provides comprehensive, gender specific primary care nursing services to Veterans within the Patient Aligned Care Team (PACT) model. Coordinates and manages care for assigned Veteran patient panels to ensure timely access, continuity, and quality outcomes.
