As Registered Nurse, Telehomecare (Status: Full-Time, Start date: May 4, 2026; Location: Any Ontario Health atHome office within the North East) you provide remote patient monitoring, health coaching, and disease-specific education to patients with complex chronic diseases, including Chronic Obstructive Pulmonary Disease and Heart Failure. Working in collaboration with the patient’s circle of care, the Telehomecare - RN focuses on enabling the patient to better manage his/her chronic disease through the support of RN monitoring and coaching.
