The Role
As part of a multidisciplinary team, the post holder is responsible for assessment of care needs and development of programmes of care, ensuring high quality delivery. The position is a fixed term 12‑months; existing NHS Lanarkshire staff may apply under secondment with prior line manager consent.
In This Key Role, You Will
Assess, plan, develop, implement and evaluate programmes of care for patients to ensure a high standard of care.
Maintain effective communication with patients, relatives and multidisciplinary team members to share information and meet patient needs.
