About the Role

We are seeking an experienced
Virtual Registered Nurse (RN)
with a strong background in
home care
to support our healthcare clients remotely. This role is ideal for an RN who is highly organized, tech-savvy, and comfortable coordinating patient care, documentation, and communication in a virtual environment.

You will work closely with home care agencies, caregivers, and patients to ensure continuity of care, compliance, and high-quality clinical support—without the need for bedside duties.

Key Responsibilities

• Conduct virtual patient assessments and follow-ups
• Review, coordinate, and update care plans in collaboration with physicians and caregivers
• Perform OASIS review and clinical documentation (as applicable)
• Monitor patient progress, identify risks, and escalate concerns appropriately
