Job Description

Registered Practical Nurse
The Patient Flow Navigator (PFN) uses clinical skill and judgement to support the flow of patients throughout the hospital. Working in tandem with the Bed Allocation Officer, the PFN liaises with the inpatient clinical unit staff to place patients in a timely manner from the Emergency Department as well as from community. Additionally, the PFN manages bed requests from outside facilities using the Criticall system. Leadership and customer service skills are key to ensuring that patients are placed appropriately.
Duties:
Facilitation of a daily bed meeting
Collaboration with IPAC
Patient flow planning and execution
Daily rounding with Inpatient Resource Nurses
Review/assessment/planning for repatriation requests
Education:
Current Certificate of Competence from the College of Nurses of Ontario
RPN diploma
Experience:
