Registered Practical Nurse Care Lead

Take the lead in resident care with S&R Group as a Registered Practical Nurse. This full-time position focuses on rehabilitation and enhancing the quality of life for residents.
As the Resident Care Coordinator and Rehab Lead, you will leverage your nursing skills to influence daily practices in long-term care. This role involves leading rehabilitation programs while providing essential support to the PSW team. Your focus on resident-directed care and quality improvement ensures optimal outcomes for all residents.
Key Responsibilities:
• Direct rehabilitation programs and support effective resident outcomes
• Collaborate with PSWs to uphold organizational standards
• Participate in home-wide quality improvement initiatives
• Assist in the onboarding and training of new team members
• Manage resident documentation and care plans accurately
Requirements:
• Current registration with the College of Nurses of Ontario
• Recent Vulnerable Sector Screening check
